WOTC SWA Survey

Thank you for participating in the Workforce Opportunity Tax Credit (WOTC) State Workforce Agency (SWA) survey sponsored by the U.S. Department of Labor (DOL) Chief Evaluation Office (CEO). The purpose of collecting the requested information is to better understand how WOTC is implemented by states throughout the nation and how states process and certify employer submissions. Please answer each question to the best of your knowledge. The survey is being administered by Economic Systems Inc. (EconSys), a third-party contractor conducting an implementation evaluation of WOTC for DOL CEO.

Burden Disclosure. Public reporting burden for this survey is estimated to average 45 minutes per response. The burden estimate includes the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and submitting the survey. This collection of information is voluntary. You are not required to respond to this collection of information unless it displays a valid OMB control number. We use your name and address for one reason only: to inform you to take the survey. After that, your answers are added to the answers of everyone else asked to do the survey. We will add up all the answers to find out how well the program is working, and individual data will not be divulged. Then Economic Systems will write a report to tell DOL on how well the program is working and what needs to be fixed to make it work better. The Paperwork Reduction Act of 1995 requires us to notify you that this information collection is in accordance with the clearance requirements of section 3507 of the Paperwork Reduction Act of 1995. DOL may not conduct or sponsor, and you are not required to respond to, a collection of information unless it displays a valid OMB number. The OMB clearance number is located at the bottom of this survey.

Participation. Your participation in this survey is required as part of your WOTC grant funding. The survey will take around 45 minutes to complete.
