The local Rural Development (RD) servicing office completes this form. RD uses this form to inform the applicant(s)/borrower(s) of their responsibility to inform the contractor of their responsibilities of incorporating the "Equal Opportunity Clause" into their contract. Applicants/borrowers will be asked to sign this form to show that they have read and understood its contents.

Submit the original of the completed form in hard copy or facsimile to the appropriate Rural Development servicing office.
