Organizations or public bodies complete this form.  Organizations or public bodies use this form to provide an annual financial statement.

 

Submit the original of the completed form in hard copy or facsimile to the appropriate Rural Development servicing office.

Enter the amount of accounts billed but not paid by customers, users, etc.  This is the gross amount before any allowances in Line 4, Allowance for Doubtful Accounts.
