FEMA is requesting approval to add two pages to the currently approved FEMA Form 90-153. There is no additional burden placed on respondents by this change. The first additional page simply consolidates the instructions that respondents already receive, but places them all in one location and are attached to the form for ease of accessibility. The second additional page is information that is already approved for collection under the clearance for collection OMB No. 1660-0061. This page provides a uniform space for respondents to provide the information.
