National Coral Reef Monitoring Program Hawai´i Resident Survey

TMKC4ET

Household or Current Resident,

NOAA uses the National Coral Reef Monitoring Program (NCRMP) to conduct periodic research of coral reefs throughout the United States and its territories. The overarching goal of this effort is to collect biological, climate, and socioeconomic information to assess changing conditions of U.S. coral reef ecosystems.

To do this we are conducting interviews with individuals who live in Hawai´i aged 18 and older. Your household was selected at random. In order for the results to be statistically representative, we would like the person 18 or older in your household who has had the most recent birthday to complete this survey.

Your participation, while voluntary, will be essential to the success of the study. Completing this survey should take less than 20 minutes of your time, and there are 2 easy ways for you to participate.

Please do one of the following before xx/xx/xxxx:

  1. Go to https://www.reefstudy.com to complete the interview online.

Please enter the passcode TMKC4ET to enter the online interview.

OR

  1. Call this toll-free number to be interviewed by phone: 1-888-978-xxxx.

Please have your passcode TMKC4ET available when you call.

As part of our protocol, please expect a reminder phone call if you do not take the survey via either method above.

Please note: the code TMKC4ET is unique to you and cannot be used by others to take the survey.

Additional information can be found at www.coris.noaa.gov/monitoring/socioeconomic, or in the Federal Register under the OMB control #0648-0646, expiration date 05/11/2021, at https://www.federalregister.gov/. The estimated burden is 20 minutes. If you have comments regarding the interview length or content, please contact Erica Towle, National Oceanic and Atmospheric Agency, National Ocean Service, Coral Reef Conservation Program, 1305 East West Highway, Silver Spring, MD, 20910, USA.

Thank you in advance for your participation in this important study!
