USED TO CARRY OUT THE REQUIREMENTS OF
TITLE IV, SECT. 453(A) OF CETA. THE JOB CORPS DATA SHEET IS THE
BASIC APPLICATION AND INFORMATION FORM USED IN THE SCREENING AND
SELECTION OF APPLICANTS. ITS PURPOSE IS (1) TO SERVE AS A BASIC
APPLICATION FORM, (2) TO DETERMINE WHETHER YOUTH MEETS ELIGIBILITY
CRITERIA, AND (3) TO PROVIDE DEMOGRAPHIC DATA FOR PROGRAM
EVALUATION.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies

1320.8(b)(3).
