USED BY THE ADMINISTRATION ON DEVELOPMENT DISABILITIES TO ASSURE
PROPER DISBURSEMENT OF THE ACCOUNTING FOR FUNDS PAID TO THE STATE
UNDER THE DEVELOPMENT DISABILITIES BASIC STATE GRANT PROGRAM. THE
DATA COLLECTED IS A RECORDING OF EXPENDITURES INCURRED OR TO BE
INCURRED UNDER THE APPROVED STATE PLAN. THE REPORTED INFORMATION IS
PRIMARILY USED FOR MONITORING AND EVALUATING EXPENDITURES OF
FEDERAL FUNDS RELATIVE TO
