SECTION 218(A)(1) OF THE SOCIAL
SECURITY ACT PROVIDES THAT THE SECRETA SHALL AT THE REQUEST OF ANY
STATE, ENTER INTO AN AGREEMENT WITH SUCH STATE FOR THE PURPOSE OF
EXTENDING THE INSURANCE SYSTEM ESTABLISHED BY THIS TO SERVICES
PERFORMED BY INDIVIDUALS AS EMPLOYEES OF SUCH STATE O ANY POLITICAL
SUBDIVISION THEREOF. THIS FORM ALLOWS THE STATE TO ASSUME THE
RESPONSIBILITY FOR REPORTING TO SSA THE AMOUNTS OF WAGES PAID TO
EMPLOYEES
