This application
form is only one of a number of reporting and recordkeeping
requirements levied on drivers and carriers by 49 CFR 391,
Qualifications of Drivers. OMB notes that DOT has been submitting
these various requirements for clearance one at a time. OMB will
give one expiration date to these various paperwork requirements.
The expiration date is January 1983, the expiration date given to
the recently approved road test. When these paperwork requirements
are submitted for renewal in October 1982, they may not be
submitted separately. At that time, the entire paperwork
requirement of this regulation must be submitted in one clearance
request with one supporting statement.

PROVIDES NECESSARY INFORMATION FOR THE
FEDERAL HIGHWAY ADMINISTRATION AND THE MOTOR CARRIER TO DETERMINE
IF THE APPLICANT IS QUALIFIED TO OPERATE A COMMERCIAL MOTOR VEHICLE
IN INTERSTATE COMMERCE PRIOR TO BEING EMPLOYED.
