collection request is approved for use through 5/31/8 on the
condition that the listing of recipient officers required at th
bottom of Form Q, Quarterly Report of Performance, is deleted. Any
resubmission of this request for extension or revision should
include discussion of the need for quarterly performance reports.
The Appropriations Committees should be contacted to determine if
the initial March 10, 1977 requirement for quarterly performance
reporting is still necessary now that the program is established.
The Cost to t Federal Government should also include printing,
mailing, etc., in addition to personnel cost.
