Abstract: THIS FORM IS USED TO COMPUTE AND COLLECT TAXES RELATED TO INDIVIDUAL RETIREMENT (I) ACCOUNTS, (II) ANNUITIES AND (III) BONDS. THESE TAXES ARE+ (I) EXCESS CONTRIBUTION TAX, (II) PREMATURE DISTRIBUTION TAX AND (III) FAILURE TO DISTRIBUTE AFTER AGE 70 1/2 TAX. THE DATA IS USED TO HELP VERIFY THAT THE CORRECT AMOUNT OF TAX HAS BEEN PAID.
