SAME CONDITION MERCHANDISE. THE FORM IS USED BY THE CLAIMANT TO
PROVIDE ON ONE PAGE INFORMATION VITAL TO CUSTOMS IN ORDER TO
APPROVE HIS DRAWBACK CLAIM. THIS IS A RECORDKEEPING REQUIREMENT
PURSUANT TO 19 CFR 22,46 WITH A RETENTION PERIOD OF 3 YEARS AFTER
PAYMENT OF SUCH CLAIMS.
