Abstract: THE DD-1884 COLLECTS INFORMATION NECESSARY TO ESTABLISH AN ANNUITY TO THE ELIGIBLE BENEFICIARY OF A DECEASED RETIRED MEMBER. THE FORM MUST BE COMPLETED AND SUBMITTED BY THE SURVIVING BENEFICIARY OF A MEMBER TO ENABLE THE UNIFORMED SERVICES FINANCE CENTER TO ASCERTAIN ELIGIBILITY AND DETERMINE OTHER CONDITIONS AFFECTING ENTITLEMENT OT AN ANNUITY.
