THIS FORM MUST BE FILED NOT LATER THAN THE FIRST DAY OF THE FOURTH CALENDAR MONTH PRIOR TO THE EXPIRATION DATE OF THE LICENSE SOUGHT TO B RENEWED UNELSS THE LICENSEE IS ONE OF THOSE RANDOMLY SELECTED TO BE AUDITED.
