REVISED TO INCLUDE QUESTIONS THAT WILL BE ASKED OF DISABLED
MEDICARE BENEFICIARIES, THEIR REPRESENTATIVES, ETC., IN CLAIM
SPECIFIC SITUATIONS TO DETERMINE IF MEDICARE IS SECONDARY PAYER FOR
SERVICES. ALSO, WE COLLECT INFORMATION TO SATISFY SPECIAL
ENROLLMENT PERIOD AND PREMIUM SURCHARGE ROLLBACK PROVISIONS FOR THE
DISABLED.
