Abstract: THE FORM IS USED BY THE MASTER OF THE VESSEL TO ATTEST TO THE TRUTHFULNESS OF ALL OTHER FORMS ASSOCIATED WITH THE MANIFEST, SUCH AS CERTIFICATES FOR SAFETY OF LIFE AT SEA (SOLAS), WATER POLLUTION, ETC. THE FORM ALSO SERVES TO RECORD INFORMATION ON TONNAGE TAX PAYMENTS IN ORDER TO PREVENT OVERPAYMENTS OF THAT TAX.
