REPORT TO TREASURY WITHIN 120 DAYS AFTER THE CLOSE OF EACH CALENDAR
YEAR, THE AMOUNT OF WITHDRAWAL AND DEPOSIT ACTIVITY BY EACH
RESPONDENT. THIS INFORMATION IS NEEDED TO COMPLETE THE REPORT TO
TREASURY BY DEADLINE.
