Form 1 is not
approved for use because DOE did not provide a burden estimate for
this collection. The remaining forms which were included in the
package submitted to OMB, (Forms 3, 9, 45, 69, and 72) are approved
for routine use by DOE through March 31, 1991. This should allow
DOE ample time to collect the remaining supporting documents and
submit them to OMB for further review.
