Terms of Clearance: This request, as amended by the 10-10-90 submission by Ron Taylor of VA to Joe Lackey of OMB, is approved. This approval is for a limited period of time, through April 1991, in light of the circumstances described below. In making its request for OMB approval of this form, VA indicated failure to comply with the requirement at 5 CFR 1320.4 that it display a currently valid OMB control number by submitting a copy of the current form in use containing no OMB control number. Continued use of this form without display of an OMB control number must cease and VA should be aware of the potential consequences described at 5 CFR 1320.5. If VA chooses to seek approval of the form beyond the April 1991 expiration date, VA must make the request for OMB approval at least 90 days before expiration of the current approval and in that request demonstrate full compliance with the requirement that the OMB control number be on the form by providing a printed copy of the form.
