STANDARD FORM 2802 AND STANDARD FORM 2802B ARE THE FORMS THE INDIVIDUAL USES TO FURNISH OPM WITH IDENTIFICATION, THE FEDERAL SERVICE INVOLVED, AND COMPLIANCE WITH OTHER PROVISIONS.
