legislative changes needed to permit elimination of the curre
requirement for a paper submission, along with electronic
submissions. Use of the form is approved in recognition that: 1) a
paper form will be required for those filers chosing to use it and
2) a paper copy still be required for all filers until current
legislation is amended eliminate the paper form for electronic
filers. You may omit printing the expiration date on this
