This information collection is approved for use through 4/94. Though OMB is approving this information collection as requested by AHCPR, it is approving it with severe reservations as to the practical utility o this particular effort and its overall usefulness in the context of AHCPR's general evaluation strategy for practice guidelines. In particular, OMB believes that severe design limitations prevent this effort from achieving AHCPR's statutory research mission. The four State sites (Michigan, Pennsylvania, Alabama, and Massachusetts) and participating PROs are inadequate in number and characteristics to control for potential confounding effects from variables such as different educational intervention strategies, patient demographics, local medical practices, and unique PRO management styles. Consequent it does not support needed inferences concerning dissemination practic or the guidelines themselves, and it does not permit valid generalization of findings to other States, PROs, physicians or patien populations nationwide. In addition, the relatively small patient and physician sample sizes provide the statistical power to recognize only large differences in response. OMB strongly recommends that prior to fielding these instruments, AHCPR increases the number of States, PROs and sites in the study, and at a minimum analyzes Medicare Part A and claims files before and after dissemination, in order to better under- stand guidelines use in the context of utilization and patient outco
