Approved for use through 12/94 under the condition that HRSA amends both the interview guides for Hospitals and Other Health Care Entities and State Licensure Agencies to collect: 1) comprehensive information regarding data elements that serve as a basis for decisions in credentialling new hires, granting privileges, making reappointments, reviewing privileges, etc.; 2) the actions actually taken in response consideration of this data; and 3) most important, the relevance of different thresholds of data and how decision making varies based on these different thresholds. For example, the matrix in D.3 of the Hospital interview guide should be expanded to delineate administrativ actions taken in response to different dollar malpractice awards (e.g. $30,000 and below or above or different levels of severity of previous disciplinarian actions.) Although the OIG previously has pursued this detailed information, HRSA should continue to evaluate these concerns. Lastly, HRSA should amend both interview guides to include questions that would enhance Federal understanding of the unintended effects of mandatory reporting of administrative, disciplinarian actions at vario levels. For example, for certain actions the existing data may be of questionable quality and under reported, because providers may resign or make other concessions in lieu of such reporting.
