PROVIDING SPECIFIC INFORMATION NEEDED FOR OPM TO MAKE COURT-ORDERED
BENEFIT PAYMENTS. THIS INFORMATION IS NEEDED TO IDENTIFY AFFECTED
EMPLOYEES AND TO CERTIFY THAT THE COURT-ORDER REMAINS IN
EFFECT.
