approved for use through 7/99 with the under- standing that: 1)
HCFA immediately must incorporate on each of the forms/instructions
the disclosure statements required by the Paperwork Reduction Act
of 1995 and its implementing regulations. HCFA must provide OMB a
copy of the amended forms/instructions for the public record; and
2) this OMB submission does not include surveyor manual
instructions or other supporting materials. Accordingly, this OMB
clearance action does not cover such materials if they exist and
are in use.

These survey forms are used by the
State Agency to record data collected in order to determine
compliance with individual conditions during fire safety surveys
and report it to the Federal Government.
On behalf of this Federal agency, I certify that
