this emergency clearance request through 8/96 to enable HCFA to
recertify Organ Procurement Organizations by June 1, 1996. OMB
approves this collection under the following conditions: 1) HCFA
immediately incorporates the disclosure statements required by the
Paperwork Reduction Act of 1995 and the implementing regulations at
5 CFR 1320; and 2) HCFA expeditiously proceeds with the standard
PRA clearance process to ensure continuity in its OMB clearance of
this submission.

This paperwork package provides
information regarding deficiencies for Organ Procurement
Organizations (OPO), as well as deficiencies noted during periodic
facility and laboratory certification surveys. This information is
used to make decisions concerning OPO redesignation,
certification/recertification of health care facilities
participating in the Medicare/Medicaid programs, and laboratories
regulated by CLIA.
