Terms of Clearance: This collection is approved as amended by ACF's revisions of 7/8/96 and on the following conditions. (1) ACF shall revise question 3 to conform with the Department's Declaration for Federal Employment. Specifically, the third and fourth question under that category which ask about charges should be dropped. In addition, the question regarding convictions of child abuse and neglect should also be dropped. (2) ACF shall delete question 4 requiring a photograph. (3) ACF shall revise the requirement regarding submission of a self-addressed, stamped postcard as agreed to with OMB. (4) ACF shall delete question 6 requiring a Career Summary. This information clearly duplicates the information collected in questions 7 and 8. (5) ACF shall revise question 8 on Work Experience to require that applicants submit employment information that is relevant to the program. ACF does not need a full employment history, accounting for periods of unemployment for the purposes of selecting Fellows under this program. (7) ACF shall add the OMB approval number, expiration date, and burden statement in accordance with the Paperwork Reduction Act of 1995. (8) ACF shall add a Privacy Act statement in accordance with the requirements under the Privacy Act. In addition, ACF cannot collect information regarding the racial/ethnic backgrounds of individual applicants for the purposes of selecting Fellows for this program. Before ACF could consider collecting such information in the future, it would have to seek an Adarand review from the Department of Justice. This collection was a violation of the Paperwork Reduction Act since it was used prior to seeking OMB approval under the Act.
