This collection
is approved in concept only under the following conditions: (1) FDA
shall outline additional steps to be taken to improve the overall
response rate, including streamlining the survey substantially to
minimize the burden on respondents. The estimated response rate of
65% is unacceptable. The steps taken to improve the overall
response rate, as well as revisions to the survey instrument
itself, shall be provided to OMB prior to conducting the survey in
the field. (2) FDA shall provide OMB a copy of all pretest results
conducted as part of this collection, along with a red-lined
version of the revised survey. OMB notes that the final instrument
was not submitted to OMB for its review until 10/10/97 which did
not provide OMB sufficient time to review the revised instrument.
OMB will continue to work with FDA in reviewing the revised survey
instrument and will provide additional comments. (3) Once OMB
approves items (1) and (2) above, FDA shall sumbit an information
correction worksheet requesting an adjustment in the burden to
reflect the actual burden associated with this survey.

Consumers will be interviewed by
telephone about their food safety awareness, knowledge, and
practices. Detailed information will be obtained about risk
perception, perceived sources of food contamination, knowledge of
particular micro-organisms, safe care label use, food handling
practices, use of food thermometers, consumption of raw foods from
animals, information sources, and perceived foodborne illness
experience. The information will be used to track progress toward
Healthy People 2000 objectives and to design education programs for
consumers, the food industry, and the media.
