NARA should report to OMB the results of the survey after the first two quarters. NARA should be able to describe the experience, including the response rate, and whether any of the questions require changes, whether new questions should be added, and whether the format is adequate. The collection is cleared on the condition that NARA:_(1) Submits to OMB the complete final survey instrument after adding introductory language at the beginning to explain the nature and purpose of the survey, how the information will be used and explaining to the respondent that their response is critical to the success of the survey's goals. This introductory information can be in the form of a covering letter to the respondent. (2) include an envelope for return, and the address to which to respond on the survey itself. (3) include a Paperwork Reduction Act notice and Privacy Act statement (if the Privacy Act applies)

The general purpose of this data collection is to initially support the business process reengineering (BPR) of the MPR reference service process and then provide MPR management with an ongoing mechanism for monitoring customer satisfaction. In particular, the purpose of the proposed MPR Customer Satisfaction Survey is to (1) provide baseline data concerning customer satisfaction with MPR's reference service process, (2) identify areas within the reference service process for improvement, and (3) provide MPR management with customer feedback on the effectiveness of BPR initiatives designed to....
