This information collection instrument
has been revised to allow its use by the Agency to verify assets of
borrowers that have requested debt settlement. To determine whether
an applicant is eligible for a loan or the appropriateness of a
servicing or debt settlement action, the Agency must document that
the debts and assets listed by an applicant are accurate. This form
has been developed for this purpose.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
