with 5 CFR 1320, the information collection is approved for a
period of three years. However, the agency must rewrite the
instructions for the form to make it clear that a visit to a State
or County FSA office is not necessary to obtain this service. In
addition, they should provide instructions that allow a respondent
to provide the correct information without a visit to an FSA
office.

This form is used by USDA/FSA
customers who want to order aerial photography products and
services from the Aerial Photography Field Office. This provides a
uniform method of collecting unique customer ordering
information.
