Abstract: The purpose of this information collection is to collect data from Medicare + Choice (M + C) organizations regarding choices that Medicare beneficiaries make as M + C enrollees. Information will be collected regarding enrollment and benefits, particularly for employment-connected individuals and will help HCFA fully evaluate the effectiveness of the M + C program. All Medicare managed care organizations will be surveyed.
