The purpose of this information
collection is to collect data from Medicare + Choice (M + C)
organizations regarding choices that Medicare beneficiaries make as
M + C enrollees. Information will be collected regarding enrollment
and benefits, particularly for employment-connected individuals and
will help HCFA fully evaluate the effectiveness of the M + C
program. All Medicare managed care organizations will be
surveyed.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
