Approved with
updated cost estimates for burden to respondents provided to OMB
via e-mail on 3/5/02. This collection expired in 1996 and has been
in use without a valid OMB number. This is a violation of the
Paperwork Reduction Act and must be reported in the ICB. In the
future, VA should take great care to ensure that all information
collections have active OMB control numbers.
