version of the form to get the full value of the electronic option
provided in accordance with the Government Paperwork Elimination
Act to raise the electronic collection from 10% at future
renewal.

The information is used to determine
the continued eligibility of a Survivor Benefit Plan (SBP), Reserve
Component Survivor Benefit Plan (RCSBP), and/or Retired
Serviceman's Family Protection Plan (RSFPP) child annuitant once
the child has reached age 18 and must attend school full-time to
continue receiving the annuity. The Survivor Benefit Division of
the Defense Finance and Accounting Service (DFAS) uses the
information to release funds.
