The SSA-1724 is used by the Social
Security Administration to collect the names, addresses and SSNs of
individuals who are requesting payment of an underpayment under
Section 204(d) of the Social Security Act. Based on this
information, SSA makes a determination of eligibility in order to
ensure proper payment. The respondents are applicants for the
amounts of an underpayment of a deceased beneficiary.
