approved as a pilot program with the following clarifications: --
All race questions must instruct the respondent to select one or
more options; -- All statements related to the Paperwork Reduction
Act should only list the agency address for comments and not list
an OMB address; and -- The agency must ensure that no form is
distributed by any means without instructions for the respondent to
follow to submit the form, including distribution through SBA's
website or other government website. Although described as a pilot
program, any changes to these forms or the administration of the
program will require OMB review prior to implementation.

SBA is piloting a small dollar loan
program ("SBA Express"). SBA Express Lenders use primarily their
own loan forms, submitting minimal information to SBA. SBA requires
the information requested to assist the agency in carrying-out its
lender, portfolio and program oversight responsibilities. The
information will also facilitate risk management.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
