SSA collects information and evidence
neeeded to issue original and replacement SSN cards. SSA uses the
information to establish enumeration records using certain elements
of personal information that uniquely identify each individual
number holder. The SSN is used by SSA to correctly identify an
individual's records, which contain the amount of wages paid, the
amount of self-employment income received by each individual and
the periods during which payments were made, as well as to pay
benefits administered by SSA.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
