Abstract: The Center for Substance Abuse Prevention has a statutory mandate to establish a national data base providing information on programs for the prevention of substance abuse which shall contain information appropriate for use by public nonprofit private entities. This form, which can be completed on-line, will obtain information about projects being nominate for inclusion on the National Registry of Effective Prevention Programs.
