This information
collection request is approved as amended by CMS and consistent
with the following terms of clearance: (1)CMS will report back to
OMB on the implementation of this collection in its 1st 6 months.
(2)CMS will carefully monitor implementation of the survey to
determine methods of maximizing the survey's practical utility and
minimizing burden on respondents. (3) CMS will evaluate the burden
of the collection and will revise its burden totals, as appropriate
at the next submission of this collection. And (4)CMS will seek OMB
approval for any focus group or market testing when more than 9
total respondents are involved consistent with the PRA.

The purpose is to collect data to
produce valid, reliable, comparable and salient quality measures to
provide a potent stimulus for clinicians and providers to improve
the quality of care they provide. This voluntary reporting
initiative in which hospitals may participate is a significant step
toward a more informed public and a means to sustain health care
quality improvement. The data is currently being collected from
hospitals by CMS. The hospitals submitting data have volunteered to
participate in public reporting. The effort places no additional
data collection requirements....
