Approved consistent with HRSA memos submitted to OMB 07/27/04 and 08/06/04. HRSA will continue to work with CMS to determine if further streamlining relating to information collected though this clearance may be achieved. In light of numerous form amendments, HRSA must review the corresponding instructions to ensure that they accurately reflect the revised forms, and HRSA must provide OMB with a statement to certify that current instructions are up to date. Additionally, HRSA must forward to OMB copies of all forms reflecting changes discussed in memos submitted to OMB including questions relating to Academic Activity level, Biological or Anti-Viral therapy, and Race and Ethnicity. HRSA will provide both forms and a certification of instructions statement to OMB as soon as possible but no later than 08/30/04.
