Title 10, USC, Sections 505, 532,
3253, 5013, and 8253 require that applicants must meet minimum and
maximum age requirements and citizenship requirements for
enlistment in the Armed Forces. If an applicant is unable to
produce a birth certificate, this information is collected using
the DD Form 372, "Request for Verification of Birth." This form is
sent to state or local agencies and is considered the official DOD
request for birth verification.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
