Abstract: Form SSA-545-BK is used when recipients of Supplemental Security Income (SSI) payments want to use available income and resources to obtain education or training but do not want to jeopardize their payments. SSA uses the information from this form to determine if the payment recipient's plan for achieving self-support can be approved under the provisions of the SSI program. The respondents are recipients of SSI payments who want to use their income/resources to pursue education or training to achieve self-support.
