The purpose of the E-mail survey is to
collect qualitative and quantitative data from health care
providers, human services providers, and consumer advocates to
learn their impressions of state-developed health promotion
materials for people with disabilities. The E-mail survey will
assess the opinions of intermediaries who impact whether and how
the materials get from the states to the consumers. The survey data
will be analyzed to evaluate awareness of state-developed materials
among professionals using the materials, assess their satisfaction
and suggestion for improvements of the materials.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).
