that several CDC sites are completing IRB review for the revised
incentive structure. Upon approval CDC shall submit documentation
that the respective IRBs have approved the use of the third

reminded that any changes to the package during the approval period
must be reviewed and approved by OMB prior to implementation.

Since 1997, the NCBDDD has been
conducting the National Birth Defects Prevention Study of risk
factors for selected birth defects. Infants with birth defects are
identified and maternal interviews, and clinical/laboratory tests
are conducted on approximately 300 cases and 100 controls per year.
Controls are selected among normal births in the same
population.
On behalf of this Federal agency, I certify that
