Producer Evaluation

Thank you for participating in the National Animal Health Monitoring System (NAHMS) Swine 2006
study. Although biological sampling is winding down and analyses of the data are not yet completed, the
NAHMS staff would like to get your input regarding your participation in the study at this time.
Your copy of the first national report from the study has been included with this evaluation. Additional
reports will be released during the next several months. National summaries of biological testing results
will be released in 2007. All national study reports are posted on our Web site at
www.aphis.usda.gov/vs/ceah/ncahs (click on Swine).
Please return this survey in the enclosed business-reply envelope (or to 2150 Centre Ave., Bldg. B, MS
2E7, Fort Collins, CO 80526-8117) at your earliest convenience. We value your opinion, so please take
