IMPORT PERMIT FORMS REVISIONS



I. Global Changes to All Forms


A. Updated the CDC website address to “http://www.cdc.gov/od/eaipp/.”


B. Revised statements to reflect language from the Final Regulations.


C. Corrected editorial errors from previous submission.


II. “Application for Permit to Import or Transport Etiologic Agents, Hosts, or Vectors of Human Disease” Form


A. Form


1. Added “( Check here if additional sheets are attached for multiple senders)” after “Organization” in Section B, Block 4.


2. Deleted “or animal” after “human” in Section C, Question #7.


B. Form (pg. 2)

1. Changed question #1 in Section E to read “Description of applicant laboratory facilities, containment equipment, and personal protective equipment ( Additional sheets attached):”


2. Added “and training” after “experience” in question #3 in Section E.


C. Guidance Document (pg. 3)


1. Deleted “or by calling the CDC fax information service at 1-888-232-3299 and requesting document number 101000.”

2. Added “Please provide a complete list of senders for each permit application submitted.” to Section B, after second sentence.

3. Added “if applicable” to last sentence in Section B.


4. Added “Please note, identification of select agents or toxins in specimens presented for diagnosis, verification, or proficiency testing has additional reporting requirements under 42 C.F.R. 73.” to last sentence, second paragraph in Section C.


D. Guidance Document (pg. 4)


1. Added “Describe the physical containment devices and personal protective equipment utilized by personnel handling the material to be imported (e.g. biological safety cabinets, respirators, face shields).” before the first sentence in Section E.


2. Added “and relevant safety training for” between “technical experience” and “personnel” in the 4th sentence of Section E.



III. “Application for Permit to Import or Transport Live Bats” Form


A. Form

1. Revised Block 1, Section C to read “Genus/Species of Bat.”


2. Deleted numbers from rows in Section C.


B. Guidance Document (pg. 3), deleted “or by calling the CDC fax information service at 1-888-232-3299 and requesting document number 101000.”
