This Paperwork Reduction Act Change Worksheet (OMB 83-C) is being submitted on OMB 0920-0650, Prevention Research Centers (PRC) Program Information System (IS). The indicated changes are to incorporate revised and specific national program indicators into the current IS and to refine some sections, from the text box format to discrete answers. The proposed changes will give the quantitative data set needed for our national program indicators and evaluation activities.
The proposed changes will not effect the burden for this Information System (IS).
Listed below are the, deletions, modifications, and additions along with a description of the justification for each proposed change.
