Other changes** OMB approved this form on 7/25/2006. HUD wishes to make minor, non-substantive changes. The OMB approval number was corrected, and web links were corrected. The questions in 20e. were revised to more accurately test applicant knowledge, and the questions were spread out to fill the empty space on the form. HUD provided an option to fax the document. Typographical and grammatical corrections were made throughout. Both the approved and the revised versions are attached.
