Request you assistance in advising and requesting OMB approval of a linking edit to the SAR-DI form that OMB recently approved. During the final testing of the procedures used by non-financial institutions to file the SAR it was discovered that an ID link had been over looked. Item 40 on the form is the source for identifying and linking the filed report to the appropriate oversight agency (see attached). When a non-financial institution such as OFAC, OFHEO, or other approved filer files a SAR they indicate their information in item 66. What is missing is a link in item 40 for these filers. The requested edit change is to add in item 40 "z Fed Other" to the existing item 40 check boxes as reflected in the attached copy of page two.
The SAR-DI is a joint form. The following OMB control numbers appear on this form FDIC-3064-0077, FinCEN-1506-0001, FRB-7100-0212, NCUA-3133-0094, OCC-1557-0180, and OTS-1550-0003.
