Changes to Instructions:

• Instructions, page 1, section 2, “What is the Fee?”, under NOTE, the word “of” has been added to the second line between the words “waiver” and “both”

• Instructions, page 2, section 3, “Applicants With Tuberculosis”, line 2, a space has been added between “Page” and “3”

• Instructions, page 2, section 3, “Applicants With Tuberculosis”, line 3, the word “is” has been changed to “are”

• Instructions, page 2, section 3, “Applicants With Tuberculosis”, the text under the bullet point has been divided into two bullet points. In line 3, the word “and” has been deleted

• Instructions, page 2, section 4, “Applicants With HIV Infection” has been added.

The numbering of sections has been changed to reflect the addition

• Instructions, page 2, number 8, “Public Reporting Burden”, line 4, “30 minutes” has been changed to “60 minutes”

• Instructions, page 2, number 8, “Public Reporting Burden”, line 13, “floor” has been capitalized

Changes to Form:

• Page 1, number 10, line 3, “Page 2” changed to “Page 3” and the following language added at the end of line 4: “If applicant has HIV infection, Page 3 of this form must be fully completed.”

• Page 5, number 10, “Page 2” changed to “Page 3” and the following language added at the end of line 4: “If applicant has HIV infection, Page 3 of this form must be fully completed.”
